Chronic back pain can be a sign of a bone fracture

Did you know osteoporosis is often an overlooked cause of chronic back pain. Bone loss can weaken bones in the spine causing spinal fractures and compressions. Chronic back pain may, in some cases, be falsely attributed to osteoarthritis, a common joint condition affecting people as they age. And, some fractures may not produce any symptoms at all. A bone density test, which measures bone mass, is used to diagnose osteoporosis.


FREQUENTLY ASKED QUESTIONS

1. What do I need from my MD?
A doctor's prescription and (an insurance referral - if applicable).

2. What is the difference between an MD prescription and an insurance referral?

An MD prescription is a Doctor's Order stating a need for Physical Therapy. It commonly suggests a treatment plan or protocol.

An insurance referral is a separate authorization from your health insurance provider that authorizes both treatment length and number of visits. It is typically attained through your primary care physician.

3. How long are the appointments?
Initial evaluations are scheduled for one hour, therapy appointments are scheduled on the half hour. Treatment sessions are generally 45 minutes in length and will vary depending on the treatment plan.

4. Will reports be sent to my MD?
Yes, only upon M.D.'s request will evaluations, progress reports and discharge summaries will be sent to your physician. Other Information and notes will be sent upon request. Medicare certifications will be sent every 30 days to your PCP.

5. Can I see the same Physical Therapist?
Patients are most often CO-treated by 2 Physical Therapists or 1 Physical Therapist and 1 Physical Therapy Assistant.

6. Are copayments necessary on each visit?
copayments are due upon arrival of each visit. Payment plans can be arranged for 80/20 and other pay plans that require payments other than copays.

7. Can I be evaluated without an insurance referral in place at the time of my visit?
Yes, you can be evaluated. However, additional visits will not be scheduled until a referral is received in our office.

8. What are your private pay rates?
$110.00 for an initial evaluation, progress note or discharge summary.
$60.00 1/2 hour visit - $85.00 1 hour visit.

9. What are the costs for custom made orthotics?
$370 including shipping (in most cases)
$185 at time of casting
Remaining balance due at orthotic fitting/distribution

10. What should I bring to my appointment?
MD prescription
Insurance Card
Legal Guardian, if under the age of 18
Loose clothing and athletic footwear

11. What happens if the weather is inclement?
Our patient's safety is our NUMBER ONE concern. It is PTI's policy to continue operations despite inclement weather unless the weather threatens to make patient or employee transportation to or from the clinic dangerous or impossible. If you do not feel comfortable driving in the inclement weather we will honor your cancellation without charge.

We will do our best to contact you if your appointment will be impacted by delayed openings, early closures or closure. Updates on any closures will be posted on our answering service as soon as they are made.
